Cancellation Policy

PEMF Waiver and Liability Release I am at least 18 years old and have read, understood, and agreed to this Waiver and Liability Release. I acknowledge that PEMF therapy is being provided by Chelsea Judge, CKG Canine and Equine Services, and I voluntarily consent to its use for my dog/horse. I understand that: PEMF is not a substitute for medical treatment, and I should consult my veterinarian for any medical concerns. PEMF is for supportive care only, including relaxation, pain relief, improved circulation, and reduced inflammation. My PEMF professional does not diagnose or treat medical conditions. Any information provided is for educational purposes, not a medical diagnosis or prescription. PEMF may cause a Herxheimer reaction or accelerate the presentation of underlying conditions. By signing, I waive, indemnify, and release CKG Canine and Equine Services from all claims, damages, and liability arising from PEMF services. I agree to cover any legal fees incurred to enforce this waiver. I understand that PEMF should not be used on animals that are pregnant, actively bleeding, have had an organ transplant, or have undiagnosed conditions. I acknowledge that Pulse PEMF does not diagnose or treat illness, and I will consult my veterinarian for any concerns. I take full responsibility for my animal’s health decisions and will keep my PEMF professional informed of any changes. I understand that PEMF is a complementary therapy, not a replacement for veterinary care. Photo Release: I grant permission for CKG Canine and Equine Services to use photos/videos of me and my animal for promotional purposes.
